Following is the du'a for leaving your home:

بِسْمِ اللهِ تَوَكَّلْتُ عَلىَ اللهِ وَلاَ حَوْلَ وَلاَ قُوَّةَ اِلاَّ بِاللهِ

Transliteration: Bismillaahi Tawak’kaltu Alallaahi Wa Laa Hawla Wa Laa Quw’wata illa billaah

Translation: "Allah’s Name we begin with, I place my (full) trust in Allah and there is no Might and Power except with Allah."

The following Du'a is for entering a home:

بِسْـمِ اللهِ وَلَجْنـا، وَبِسْـمِ اللهِ خَـرَجْنـا، وَعَلـى رَبِّنـا تَوَكّلْـنا

Transliteration: Bismillaahi walajnaa, wa bismillaahi kharajnaa, wa ‘alaaRabblnaa tawakkal naa

Translation: "In the Name of Allah we enter, in the Name of Allah we leave, and upon our Lord we depend."

Then say Assalaamu ‘Alaykum to those present in the house.

Du'a for Leaving and Entering a Home
